Where can I find the Customer Experience Survey?

Thanks for your interest in taking the customer experience survey! If the survey is available, you will see it indicated either on your printed receipt or in your Chick-fil-A® App.  

  • Receipt: If the survey is available, you can find the serial number or Survey ID Code printed at the bottom of your receipt. Visit the My CFA Visit page to take the survey. Note: Not every receipt includes an invitation to take the survey. 
  • Chick-fil-A App: If the survey is available, please open the Chick-fil-A App and click on the “For You” icon at the bottom of the screen. If available, you will see the invitation to complete the survey.
